For the Salt Lake Valley Co-op, please find the form you need from the list below.

All PDF forms have the form fill feature which allows you to fill in the content on your computer.  If you would like to save the completed form on your computer, you MUST turn it into another PDF with a different name using a PDF converter program.

We recommend Primo PDF. It is free, easy to use and stores itself conveniently on your computer as a printer.  Select "print" in Adobe in your filled out document and choose the Primo PDF printer, give it a new name and tell it where to save the document.

Younger Co-op (Salt Lake Valley)

The Salt Lake Valley Younger Co-op is a traditional parent led co-op serving children between newborn and 3rd grade. It meets at The Fellowship Church (615 East Sego Lily Dr, Sandy) and runs on Fridays from 9:00 - 11: 00 am.

Co-op classes are split into groups; nursery (ages 0-2), preschool (ages 3&4), K-1st grade and 2nd -3rd grade. We split our co-op into 2 semesters, alternating between science and history/geography curriculums.

Parental responsibilities include but are not limited to the following:

  • $25 class fee per child 
  • You will be required to teach/help in one of your child’s classes on a rotation basis
  • You must be a UTCH member

The curriculum for the Younger Co-op for the 2013/2014 school year will be as follows:

Spring - All about the Farm, All about my Body
K-1st
Fall - Five in a Row Vol. 1
Spring - Teaching Science through Favorite Picture Books
2nd & 3rd
Five in Row Vol. 2/ Life in Plymouth Colony History Pockets/ Lifesaver Science Lessons (various topics)
